Location: Bohemia, NY 11716| Schedule: Full time 35/hours | Status: Non-Exempt |Salary/Hourly rate Range: $24.37 to $26.37/hr

The Opportunity

Family and Children's Association (FCA) is an organized and detailed Case manager for our Clinical Services Division.

Responsibilities

• Conduct community outreach and provide education on Adverse Childhood Experiences (ACEs), their long-term impact, and strategies to reduce risk factors.

• Attend community events and deliver presentations to schools, community centers, and organizations to increase awareness of program services and referrals.

• Provide individual and family case management services to connect clients with community resources and referrals to support their needs

• Collaborate with internal programs and external service providers to support client progress and continuity of care.

• Conduct intakes, complete assessments, and develop and monitor individualized care plans.

• Participate in interdisciplinary team meetings, case conferences, and trainings.

• Maintain accurate and timely client records, reports, and documentation in compliance with agency and regulatory requirements.

• Perform additional duties and projects assigned to support program operations.

Qualifications

• Minimum 1 year of experience working with youth, families, or in-community-based social services (internship experience will be accepted), required.

• Bachelor's degree in Social Work, Psychology, Human Services, Public Health, or related field; preferred.

• Preferred: Bilingual in Spanish and English.

• Knowledge of trauma-informed care, Adverse Childhood Experiences (ACEs), and behavioral health prevention preferred.

• Ability to provide services in office, community, and telehealth settings.
