This role is based within our Hospice Center, where you will collaborate with a multidisciplinary team of healthcare professionals, including physicians, nurses, and fellow therapists. Key Responsibilities\nIn the role of Senior LCSW, you will be expected to:

- Conduct thorough clinical assessments and formulate individualized treatment plans that reflect evidence-based practices and patient preferences. \n
- Implement and oversee therapeutic interventions such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT) tailored to the unique needs of each client. \n
- Maintain accurate clinical documentation aligned with HIPAA standards to ensure compliance and facilitate quality care delivery. \n
- Collaborate effectively with a multidisciplinary treatment team to coordinate comprehensive care and support for patients, sharing insights on their mental health needs. \n
- Engage in regular case consultations to enhance clinical skills and share knowledge with peers in a supportive environment. \n
- Facilitate educational sessions for patients and their families, providing them with resources concerning mental health issues and treatment pathways. \n
- Stay current with the latest research and developments in mental health to continuously improve care methodologies. \n
In this role, you will primarily work with patients receiving hospice care who are facing end-of-life issues, potentially including chronic illness, grief, and significant life transitions. Your expertise will help patients and their families navigate emotional challenges and foster a sense of peace and closure. This requires not only clinical skills but also a deep sense of empathy and understanding of the complex dynamics surrounding end-of-life care.
Qualifications\n- \n
- A Master’s degree in Social Work from an accredited university. \n
- A valid and active Licensed Clinical Social Worker (LCSW) license in the state of California. \n
- A minimum of three years of relevant clinical experience, with demonstrated proficiency in providing psychotherapy services. \n
