Responsibilities
- Conduct comprehensive speech-language pathology evaluations for residents within the SNF setting and develop individualized treatment plans.
- Provide therapy to address communication, cognition, and swallowing disorders; implement goals across bedside and daily living activities.
- Document evaluations, treatment plans, progress notes, and outcomes in Rehab Optima (Net Health) in a timely, compliant manner.
- Perform swallowing screenings and dysphagia management, including dietary recommendations and modifications as appropriate.
- Collaborate with an interdisciplinary team (nursing, OT, PT, physicians) to optimize resident outcomes and support discharge planning.
- Educate residents and families about communication strategies, swallowing safety, and progression of therapy while ensuring privacy and regulatory compliance.

Requirements
- Active California license as a Speech-Language Pathologist
- More than 1 year of SNF experience
- Knowledge of Rehab Optima (Net Health) software
- Ability to work in a day shift within a travel assignment in a SNF setting
- Strong communication and teamwork skills; ability to work autonomously when needed
