Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for senior clients, considering their emotional, mental, and social factors to develop tailored care plans.

• Collaborate with clients and families to set personalized goals, actively guiding them through necessary resources and services.

• Provide counseling and emotional support to seniors navigating transitions such as health issues, loss, and lifestyle changes.

• Advocate for clients' rights and ensure their access to vital community resources, including healthcare, social activities, and support groups.

• Participate in interdisciplinary team meetings to develop coordinated strategies for enhancing client care, making recommendations based on clients’ assessments.

• Perform regular follow-up visits, both virtual and in-person, to monitor clients’ progress, ensuring that their evolving needs are met.

• Facilitate improved communication among family members to address conflicts and support the senior client's care strategies effectively.

• Educate clients and families on available services and community programs, increasing their awareness and promoting social engagement.

• Maintain accurate documentation of assessments, care plans, progress notes, and interactions in compliance with organizational standards.

• Continuously update knowledge of social work practices, policies, and regulatory standards to adhere to ethical requirements.

Qualifications\n

Compensation and Benefits\n
