Work Environment
This role is based within a community health clinic and is conducted in a fully remote setting, providing an excellent opportunity to reach clients in various locations. Key Responsibilities
- Conduct thorough and ongoing assessments to create tailored treatment plans that address the unique needs of each patient.
- Maintain open communication with supervisors, direct care staff members, families, and other relevant individuals to provide updates on patient progress and conditions.
- Schedule and coordinate appointments, home visits, and observation sessions to monitor patient improvement and adherence to their treatment plans.
- Perform home visits as required to assess patients in their natural environments, ensuring comprehensive care.
- Oversee the effective implementation of treatment plans, adjusting strategies as necessary to meet evolving patient needs.
- Engage in Interdisciplinary Team Meetings to discuss patient care, share insights, and collaborate on treatment approaches.
- Document patient progress through monthly and quarterly notes, ensuring accurate and timely updates on treatment outcomes.
- Lead training sessions to ensure that staff members are equipped with the skills and knowledge for implementing program plans effectively.
- Conduct follow-up home visits to provide ongoing support to patients, ensuring that they continue to receive the assistance they need.
- Act as a liaison between patients, the community, and mental health services, leveraging partnerships to facilitate better access to care.
Patient Population / Client Focus

You will work with individuals from diverse backgrounds and with varying issues, allowing you to apply your expertise in a range of contexts, including family therapy, trauma recovery, and substance misuse treatments. Qualifications
- A Master’s or Doctoral degree in Counseling, Psychology, or a related mental health field is required.
- Active and unrestricted licensure as a Licensed Professional Counselor (LPC) in the state of Texas.
- A minimum of 2 years of counseling experience in a clinical setting is essential to provide quality care.
- Strong ethical and legal understanding pertinent to the practice of mental health counseling.
- Exceptional interpersonal and communication skills to build rapport with clients and stakeholders alike.
- Proven ability to collaborate effectively within a multidisciplinary healthcare team.
- Familiarity with Electronic Medical Record (EMR) software and other relevant technologies is critical.
Preferred Experience
- Expertise in specialized areas such as family therapy, couples therapy, trauma-informed practices, or substance misuse treatment is highly preferred.
- Experience with diverse populations and a strong cultural competency are advantageous.
