Full-Time Physical Therapist (PT) – Home Health
Deer Meadows Home Health
Primary Territory: Northwest Philadelphia & Montgomery County, PA
Deer Meadows Home Health is seeking a compassionate and motivated Full-Time Physical Therapist (PT) to join our home health team!
This is a great opportunity for a PT who enjoys working independently, building meaningful relationships with patients, and providing quality therapy in the comfort of patients’ homes.
Position Highlights
Full-Time Physical Therapist opportunity
Primary treating territory: Northwest Philadelphia & Montgomery County
Home health setting
Homecare Homebase (HCHB)
Supportive and collaborative team environment
Opportunity to make a direct impact on patients’ independence and quality of life
Qualifications
Current Pennsylvania Physical Therapist license
Home health experience preferred
Experience with Homecare Homebase preferred
Strong clinical assessment and communication skills
Ability to work independently while collaborating with the interdisciplinary care team
Reliable transportation and ability to travel throughout the assigned territory
Responsibilities
Provide skilled physical therapy services to patients in their homes
Complete evaluations and develop individualized plans of care
Provide therapeutic interventions focused on mobility, strength, balance, safety, and functional independence
Educate patients, caregivers, and families
Coordinate care with physicians, nursing, therapy, and other members of the home health team
Complete timely and accurate documentation using Homecare Homebase
