Service Facilitator (CCS)

We are seeking a dedicated Service Facilitator to support individuals with mental health challenges through a person-centered, recovery-focused approach. Key Responsibilities Include Conducting functional assessments and developing individualized service plans in collaboration with clients, families, and providers. Coordinating and authorizing community-based services and facilitating recovery-focused team meetings. Connecting clients to community resources and support systems. Maintaining accurate documentation, completing reports, and collaborating with treatment teams and community partners. Qualifications Master's degree in a human services-related field such as Social Work, Psychology, Counseling, or related disciplines. Valid driver’s license and reliable transportation for community travel. Ability to work flexible hours, including occasional evenings, as needed. Preferred Experience Providing mental health, behavioral health, therapy, case management, or care coordination services. Working with children, families, and adults with complex needs in recovery-oriented, trauma-informed, or person-centered settings.
