How you'll contribute As a Nurse Navigator (RN), you will serve as a patient's guide through the healthcare system, helping coordinate care, remove barriers, and improve the overall patient experience. Job Duties * Serve as the primary point of contact for patients and families * Coordinate appointments, testing, treatments, and referrals * Provide education about diagnoses, treatment options, and available resources * Assess barriers to care and connect patients with community resources and support services * Advocate for patients and helping ensure timely, appropriate care across the continuum * Provide hands-on patient care including but not limited to vitals, patient intake, in-clinic procedures * Cover other clinics as necessary * Other duties as assigned What we offer Fundamental to providing great care is supporting and rewarding our team. Qualifications * Standard ADN and BSN preferred * Outpatient Clinic preferred * ACLS and BCLS certifications required.
