Home Health Physical Therapist
This is a 13-week contract position.
Day shift - 5x8 Hour schedule (08:00 - 17:00)
We are seeking an experienced Home Health Physical Therapist to provide high-quality care to patients in a variety of clinical settings, including med/surg, cardiac, and ortho cases. The therapist will conduct patient assessments, develop and implement individualized treatment plans, and ensure proper documentation of patient progress.
Responsibilities
- Conduct thorough assessments of patients in their homes.
- Develop comprehensive treatment plans to enhance patient mobility and function.
- Implement effective therapeutic interventions utilizing adaptive devices and modalities.
- Educate patients and families on home exercise programs and safety protocols.
- Document patient progress and outcomes in compliance with regulations.
- Collaborate with interdisciplinary teams to optimize patient care.
Required Experience / Certifications / Licensure
- Minimum of 1 year of experience in home health care.
- Valid state licensure as a Physical Therapist is mandatory.
- Current BLS certification required.
- OASIS training preferred.
- Experience with Medicare, Medicare Advantage, and commercial insurance documentation.
