• Shift #1 - 4pm-12am
• Shift #2 - 12am-8am
• Nature Of Work:
• This position is in the Social Services Case Management Job Family. Incumbents provide a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an individual's or family's comprehensive needs in physical health, mental health/behavioral, special developmental or intellectual diagnoses, and/or family stability needs resulting from abuse and/or neglect. Incumbents serve as a lead worker to lower level staff. Responsibilities may include: risk assessment, permanency planning, visitations with clients, representing the department at various community funding meetings

Distinguishing Characteristics:

• This position is responsible for providing process management of individual and/or family needs in human services areas and serve as a lead worker to subordinate staff members.

Typical Class Responsibilities:

• (These responsibilities are a representative sample; position assignments may vary).
  Interviews individuals/families to gather data involving their social, health, emotional and economic needs; and assesses needs of individuals/families.
• Provides support to subordinate human services staff members; assists staff members with court paperwork and documentation; reviews documentation prior to submission to ensure program compliance.
• Investigates allegations of abuse, neglect, and financial exploitation by conducting home visits and assessing client safety.
• Consults with peers to ensure caseload management and proper staffing of ongoing caseloads; makes caseload service and placement recommendations.
• Participates in program planning and evaluation meetings regarding program change; funding; and evaluation.
• Performs other duties of a similar nature or level.
