Senior Care Case Manager & Care Advocate

Join Our Team! We are seeking a dedicated and detail-oriented Case Manager to support residents’ quality of life through effective care coordination and advocacy. This role works closely with residents, families, staff, and external providers to ensure care plans are person-centered, compliant, and compassionate. If you’re passionate about senior care and enjoy a collaborative work environment, we invite you to apply! Qualifications Bachelor’s Degree in Human Services, Case Management, Social Work, or a related field. Previous experience in case management within assisted living or a similar healthcare environment. Strong interpersonal, communication, and advocacy skills with the ability to build rapport across all levels. Proficiency in electronic health record (EHR) systems and standard office software. Ability to work independently, exercise sound judgment, and collaborate effectively with interdisciplinary teams.
