Contract | Full-Time | Onsite | Monday–Friday, 8:00 AM – 5:00 PM | 13-Week Assignment

Key Responsibilities

• Provide skilled physical therapy services to patients within the Anchorage municipality territory
• Conduct patient evaluations, develop individualized treatment plans, and deliver therapeutic interventions in the home health setting
• Complete OASIS assessments accurately and in accordance with regulatory requirements
• Maintain productivity standards within a 40-hour work week
• Document all patient care activities and progress in the electronic medical records system

Required Qualifications

• Active Physical Therapist (PT) license in good standing
• Minimum of 1 year of home health experience
• Minimum of 1 year of OASIS experience
• Ability to manage a full-time caseload with a 40-hour guaranteed work week

Preferred Experience / Skills

• Experience with HCHB (Homecare Homebase) electronic medical records system
• Familiarity with home health productivity and point-based documentation systems
