Position Summary

The Clinical Patient Access Specialist conducts structured patient outreach to established or assigned patients, administers a standardized sexual health questionnaire, documents responses, identifies access barriers, and supports appropriate follow-up and connection to care. The role may be filled at the RN, APRN, or physician level. 

Essential Duties and Responsibilities

· Conduct confidential sexual health assessments, obtain comprehensive histories, perform risk assessments, and complete appropriate clinical evaluations.

· Provide evidence-based education, counseling, prevention planning, and shared decision-making in a culturally responsive, medically accurate trauma-informed, and LGBTQ+ affirming manner.

· Conduct appropriate follow-up communications, including outreach for results, treatment, adherence, missed appointments, and retention in care.

· Support patient access by identifying barriers and providing practical solutions. 

· Accurately document all patient call attempts and outcomes in the designated Call Tracker each workday. 

· Meet the daily call-volume expectations and complete assigned calling shifts in accordance with the current Patient Call Policy, including all applicable scheduling, documentation, and patient-contact requirements.

· Support relationship-building efforts with community organizations, healthcare partners, industry groups, and other stakeholders.

· Collaborate with Compliance, Finance, Human Resources, and Information Technology, to achieve organizational objectives.

Referral, Linkage and Care Coordination

· Refer and link patients to STI and HIV testing services at Nashville CARES and/or CARES 360 Clinic (633 Thompson Lane) in phase 1, and to the same provider(s) at 100 Oaks once the on-site STI testing facility is open (phase 2). Upon request, a patient may also be referred to a different testing service. 

· Coordinate referrals for new-patient establishment and for continued/ongoing care with appropriate providers, including the Vanderbilt Comprehensive Care Center (VCCC), Nashville CARES / CARES 360 Clinic, and other community medical providers.

· Facilitate warm hand-offs, assist with appointment scheduling, and confirm that patients are successfully connected to the referred service or provider.

· Serve as a consistent point of contact who helps patients navigate the sexual health and HIV/STI care continuum.

· Identify barriers to care (e.g., cost, transportation, scheduling, stigma, or fear), and refer to Nashville CARES, the VCCC, or additional community partners for resolutions and service provisions.

Required Qualifications

· Qualification for at least one selected tier: (a) active, unrestricted RN license; (b) active, unrestricted APRN/NP authority and applicable national certification; or (c) active, unrestricted physician license. 

· Ability to use Microsoft 365, video-conferencing tools, call-tracking systems, etc. 

· Successful completion of required background, exclusion, credentialing, education, employment, and drug-screen processes, as applicable.

· Carry out additional duties as assigned. 

· The candidate must be able and willing to travel locally, regionally, and nationally to support marketing, communications, and community engagement activities. 

· Excellent communication and interpersonal skills.

· Strong typing and data entry skills.

· Positive attitude; energetic, flexible, and professional with exceptional phone etiquette.

· Ability to operate efficiently in a fast-paced environment while fostering positive patient relationships.

Documentation, Confidentiality, & Compliance

· Maintain accurate, timely, and confidential documentation in the electronic health record EHR) and/or patient navigation system, including but not limited to eClincialWorks.

· Adhere to HIPAA and all applicable federal and state confidentiality requirements, including 42 CFR Part 2 where applicable.

· Practice strictly within the scope of the individual’s professional license and applicable laws.

· Support program data collection and reporting requirements (e.g., quality, grant, Ryan White, and/or 340B program metrics, as applicable).

Preferred Qualifications

· One to three years of experience in patient outreach, ambulatory care, telehealth, sexual health, HIV care, public health, care coordination, or patient access.

· Spanish/Bilingual speaker 

· Experience serving people living with HIV, LGBTQ+ communities, communities of color, and patients affected by stigma or access barriers.

· Knowledge of PrEP, PEP, Doxy-PEP, HIV/STI prevention and treatment, Ryan White programs, FQHCs, or the 340B environment appropriate to the selected tier.

· Demonstrated ability to build rapport and provide culturally responsive, nonjudgmental, and affirming support in an ethnically, culturally, and racially diverse environment.

· One to three (1–3) years of experience working in a setting that partners with a nonprofit organization or Covered Entity participating in the 340B Drug Pricing Program.

· Strategic thinking and the ability to align clinical activities and initiatives with organizational goals.

· Experience working in an ethnically, culturally, and racially diverse environment.

· Ability to establish positive rapport, build relationships, and provide culturally sensitive patient care.

· Experience working with companies or nonprofit organizations that provide direct care or services to patients living with HIV and/or patients who identify as part of the LGBTQ+ community.

· Foundational knowledge of, or willingness to develop knowledge regarding, key concepts and terminology relevant to LGBTQ+ populations, including pronoun usage, sex, sexual orientation, gender, gender identity, and intersectionality.

Performance Expectations

· Complete assigned outreach and follow-up within established timeframes.

· Maintain timely, accurate, and complete documentation with no unaddressed clinical escalations.

· Meet daily and weekly quality and productivity targets.

· Demonstrate respectful, affirming communication and consistent compliance with privacy and patient-safety procedures.

· Benefits include competitive compensation ($45 - $80 per hour). Additional certification can increase the hourly rate incrementally, subject to NPS Healthcare's determination.
