The Re-Entry Specialist provides case management, advocacy, resource coordination, and transition support to New Freedom members. Working closely with members and multidisciplinary treatment teams, this position helps identify needs, develop individualized plans, coordinate services, and reduce barriers related to housing, healthcare, benefits, transportation, employment, education, and financial stability.

• Developing and monitoring individualized service and transition plans
• Coordinating housing, healthcare, benefits, transportation, employment, education, and community resources
• Helping members navigate referrals, applications, appointments, and eligibility processes
• Supporting recovery, self-sufficiency, and successful community reintegration
• Collaborating with treatment teams and community providers to address member needs and barriers
• Maintaining timely, accurate, and compliant case management documentation
• Supporting approved appointments, community visits, and transportation activities when assigned

• Members are connected with appropriate services and community resources
• Individualized goals and transition plans reflect each member’s needs and strengths
• Barriers to housing, employment, healthcare, benefits, and transportation are addressed proactively
• Members strengthen their independence, self-advocacy, and community support networks
• Documentation, follow-up, and referrals are completed accurately and on time
• Strong collaboration supports continuity of care and positive reentry outcomes

• High school diploma or GED (Associate or bachelor’s degree in Social Work, Behavioral Health, Psychology, Human Services, Criminal Justice, Public Health, or a related field preferred)
• Must be 21 years of age or older
• Behavioral Health Technician designation based on qualifying education and/or experience
• Experience in case management, care coordination, social services, behavioral health, recovery support, reentry services, or human services (Two or more years of relevant case management, care coordination, reentry, recovery, or behavioral health experience preferred)
• Knowledge of community resources, individualized planning, service coordination, documentation, and member advocacy
• Strong communication, organization, time management, and problem-solving skills
• Ability to manage multiple members, referrals, deadlines, and follow-up responsibilities
• Ability to maintain confidentiality and appropriate professional boundaries
• Proficiency with Microsoft Office, Electronic Health Records, case management systems, and related technology (Experience using KIPU or a comparable Electronic Health Record or case management system preferred)
• Case management training, certification, or demonstrated experience preferred
• Relevant lived experience with recovery, reentry, behavioral health services, or justice-system involvement and Peer and Recovery Support Specialist certification preferred
• Experience serving justice-involved individuals or populations facing housing, employment, healthcare, benefits, or transportation barriers preferred

This position works across outpatient behavioral health, recovery, reentry, residential, professional office, and community-based environments. Responsibilities may include member meetings, service coordination, community appointments, transportation, and other off-site activities. Scheduling may occasionally include early mornings, evenings, weekends, or holidays based on program and member needs.

• Periods of sitting, standing, walking, and moving between work locations

• Occasional local travel and accompaniment of members to approved appointments or services
• Occasional lifting, carrying, pushing, or moving of materials weighing up to 30 pounds
• Ability to observe changing circumstances and communicate concerns appropriately
• Ability to safely operate a company vehicle when transportation duties are assigned
