Mental Health Case Manager
Chicago | Contracted 
We're hiring a Mental Health Case Manager to help clients connect with the behavioral health, housing, medical,
and social services they need to thrive. You'll be a trusted advocate, navigator, and coordinator across our
clients' care networks.
What You'll Do
● Assess client needs across behavioral health, medical, housing, and social services.
● Build individualized case-management plans aligned with treatment goals.
● Connect clients to community resources and confirm successful linkage.
● Coordinate with hospitals, providers, schools, and community organizations.
● Advocate for clients facing barriers to needed services.
What You'll Bring
● Meets the qualification level required for the assigned Case Management service.
● Bachelor's degree in social work, psychology, sociology, human services, or related field preferred.
● Experience coordinating behavioral health, healthcare, or social services.
● Knowledge of Chicago and Cook County community resources.
Nice to Have
● Experience with Medicaid behavioral health services.
● Background in discharge planning, housing navigation, or public benefits.
