Coverage Area Clark, Crawford, Floyd, Harrison, Jefferson, Scott, and Washington Counties Registered Nurse (RN) - Home Health Practice Code: 12360 Coverage Area Clark, Crawford, Floyd, Harrison, Jefferson, Scott, and Washington Counties Position Overview We are seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team. The ideal candidate will be responsible for providing high-quality patient care in the home setting while supporting patients and families through education, assessment, and coordination of care. Schedule & Productivity Schedule: Monday through Friday, 8:00 AM - 4:30 PM Weekly Hour Guarantee: 40 hours Productivity Expectation: 30 points per week On-Call Requirement: 1-2 days per week and one weekend per month Mileage Reimbursement Mileage is reimbursed between patient visits, from the first patient to the last patient of the day. Key Responsibilities Complete OASIS assessments and documentation Provide wound care and patient monitoring Perform catheter care and maintenance Collect laboratory specimens as ordered Deliver disease process education to patients and caregivers Provide medication education and management supportPerform PICC line care and patient/caregiver education Conduct LPN and home health aide supervisory visits Participate in interdisciplinary team case conferences Collaborate with physicians and care team members to ensure optimal patient outcomes Required Qualifications Active, unrestricted Registered Nurse (RN) license Current CPR certification Minimum of one year of RN experience Strong clinical assessment and patient education skills Ability to work independently and manage a caseload effectively Preferred Qualifications Home Health experience Homecare Homebase (HCHB) experience OASIS knowledge and assessment experience #J-18808-Ljbffr
