Home Health Caregiver Position

Job Summary:

Modani Care is seeking a compassionate and dedicated Home Health Caregiver to join our team in Boonville, MO. As a Home Health Caregiver with Modani Care, you will provide exceptional in-home care services to seniors and adults who need support with daily living tasks, while promoting independence and dignity. About the Role:

As a Home Health Caregiver with Modani Care, your responsibilities will include:

Providing personalized care services to clients in their homes, including assistance with daily living activities, medication management, and mobility support
Maintaining accurate and detailed records of client care and services provided
Collaborating with the care team to develop and implement personalized care plans that meet each client's unique needs
Delivering exceptional customer service to clients and their families, addressing concerns and providing reassurance and support
Maintaining a safe and clean environment for clients, adhering to all safety protocols and infection control measures

Requirements:

Ability to lift and transfer clients safely and assist with mobility needs
Effective communication and interpersonal skills
Ability to work independently and as part of a team
Reliable transportation and ability to travel to client homes
