Emergency Medicine - Clinical Pharmacist - Full-Time Summit, NJ, United States The Clinical Pharmacist delivers comprehensive, patient-centered pharmaceutical care across a variety of specialty areas, including Behavioral Health, Critical Care, Emergency Medicine, Infectious Disease, Pediatrics/Neonates, Oncology, Inpatient Transitions of Care, Investigational Drug Services, and Opioid Stewardship. The role is responsible for reviewing and optimizing medication therapy, performing medication reconciliation, monitoring therapeutic outcomes, managing adverse drug events, and providing patient and caregiver education. The pharmacist collaborates with interdisciplinary teams to develop individualized care plans, participates in clinical rounds, and provides drug information and pharmacotherapy recommendations. Responsibilities Participate in daily patient care in the emergency department, providing recommendations for acute management and supporting medication history/reconciliation activities. Participate in drug therapy management of medical emergencies, including providing clinical guidance and medication preparation support during acute care resuscitation, code stroke, STEMI, sepsis, neurologic disorders, and trauma activations. Respond to emergency situations including cardiac arrest (Code Blue), post-arrest targeted temperature management, acute ischemic stroke (Code Stroke), and rapid sequence intubation. Provide drug information, including management of acute toxicologic emergencies, and education. Provide therapeutic drug monitoring recommendations as appropriate. Prevent, detect, monitor, document, and report adverse drug reactions and medication safety events. Participate in quality, safety, and other project work related to emergency medicine. Conduct medication use evaluations and research projects in collaboration with other healthcare professionals to identify areas of medication use optimization with publication and/or presentation at local and/or national meetings. Participate in site and system-wide ED committees. Precept pharmacy residents and pharmacy students. Qualifications Graduation from an accredited School of Pharmacy with PharmD required. Current active registration and licensure with the State of New Jersey Board of Pharmacy required. Must have previous hospital pharmacy experience. Nationally recognized certifications in specialized areas (e.g., BCPS) preferred. Active engagement and participation in committees. Experience with PACT projects that heavily impact/involve specialty area. Trainer in a specialty area and expert among peers.
