Homecare Hospice CNA PRN

Schedule: PRN, minimum requirement of 2 shifts in a 4 week period (Monday-Sunday, anytime) This position provides personal care and home management tasks at the patient’s residence, under the supervision of a RN Case Manager. Essential Duties Follows the nurse aide care plan to provide compassionate care to the patient in the areas of personal hygiene, elimination, feeding, and mobility. Reports changes in patient’s condition, concerns about patient and/or family to appropriate nursing staff in a timely fashion and participates in interdisciplinary team conferences as necessary. Works collaboratively with the IDG and other departments in achieving patient care goals. Documents patient care according to agency policy and procedures; documentation is clear, complete, concise, and timely. Responsible for professionally managing time, people, medical supplies, and technology. Establishes and adheres to a timeframe for patient visits in coordination with patients, families and facility staff members. Maintains a clean, safe and comfortable environment for the patient. Teaches and/or reinforces home safety, energy conservation and sanitation. Provides light housekeeping tasks. Assists with, or provides, meal preparation and related tasks. Demonstrates excellent communication skills and creates successful interpersonal relationships with patients and families. Complies with organizational nurse aide standards. Participates in staff training and education programs. Average of 4-6 visits a day. Challenges In hospice care, you are dealing with grief.
