Remote Care Coordinator for Complex Health Needs

Texas Health Institute

UnitedHealthcare is seeking a Care Coordinator to serve as the primary care manager for a panel of members with chronic and complex health care needs. You will support the care team and coordinate medical, behavioral, and social drivers of health to promote optimal outcomes.
