Join us today, and experience a career well lived.Katy, TX Salary: $75,000–$80,000 annually (based on experience) Schedule: Monday–Friday | Approximately 8:00 AM–5:00 PM Work Setting: Field-Based / In-Person - This is not a remote role.Your RoleJoin a collaborative nursing team supporting approximately 35 individuals in both group homes and host homes. This is a fast-paced nursing case management role where you'll coordinate healthcare services, complete clinical assessments, and serve as a trusted resource for individuals served and direct support staff. This is a field-based position that requires regular travel from home to home using your personal vehicle. Mileage is reimbursed for work-related travel.Responsibilities include:Conduct nursing assessments, develop and implement healthcare plans, and respond to changes in condition.Communicate with physicians, hospitals, pharmacies, and other healthcare providers to coordinate care.Ensure physician orders are accurately documented and carried out.Provide medical oversight, training, and guidance to direct support staff.Participate in person-centered service planning and advocate for quality healthcare outcomes.Travel between residential locations using your personal vehicle (mileage reimbursement provided).Maintain timely clinical documentation and meet weekly documentation expectations.Participate in a rotating after-hours on-call schedule with one other nurse.
