- Document progress and coordinate with the care team to optimize outcomes.
- Provide patient and family education on home exercise programs and safety.

Requirements
- Active physical therapy license in Oregon
- Experience in outpatient physical therapy (outpatient setting preferred)
- Ability to work effectively in a hospital-based outpatient setting
- Strong communication and collaboration skills
- Comfortable working as part of an interdisciplinary team
