Role Summary The Case Manager plays a key role in helping individuals with intellectual and developmental disabilities access, coordinate, and monitor services in the community. This position involves developing and maintaining individualized support plans, ensuring service quality, and collaborating with participants, families, providers, and community partners. The ideal candidate is detail-oriented, empathetic, and skilled in problem-solving, communication, and coordination across multiple service systems. Key Responsibilities • Serve as a liaison between waiver providers, state agencies, and community partners to support individuals with disabilities. • Develop, update, and review Person-Centered Individualized Support Plans (PCISP) in collaboration with individuals, families, and support teams. • Monitor service delivery and utilization through home visits, calls, and meetings to ensure services align with the PCISP. • Ensure participant health, safety, satisfaction, and quality of care outcomes. • Complete and process annual level-of-care determinations and maintain compliance with state and organizational documentation standards. • Facilitate in-person meetings, complete monitoring checklists, obtain necessary signatures, and ensure timely documentation and uploads. • Submit and follow up on incident reports per state-approved processes. • Maintain accurate and organized case files per state and program standards. • Disseminate Service Authorizations (SA) and required documentation to the participant and Individual Support Team (IST). • Build and strengthen natural and community supports for each participant. • Conduct community outreach to promote awareness of Columbus's Case Management services. Required Qualifications • Bachelor's degree in a Human Services field. • Minimum of one (1) year of experience working with individuals with developmental disabilities. • High level of initiative, self-direction, and accountability. • Must reside in the assigned territory (exceptions may be considered based on business needs). • Valid driver's license, automobile insurance, and access to reliable transportation. • Willingness to travel regularly and work occasional evenings or weekends as needed. Preferred Qualifications • Knowledge of social, economic, and rehabilitative services and community resources. • Experience with person-centered planning and case management systems. • Strong organizational, analytical, and communication skills. • Proficiency with Microsoft Office 365 and electronic documentation systems.
