How you’ll make an impact in this role Lead the strategic direction and oversee the daily operations for a critical, high-impact case management program. Drive high-quality patient outcomes by evaluating services, driving the development of personalized care plans, and achieving performance excellence. Collaborate with physicians and a diverse multidisciplinary team to champion evidence-based care and reduce clinical variation. Serve as the go-to case management expert, aligning resources and specialized knowledge to support key, system-wide healthcare initiatives. Resolve complex operational challenges, ensuring a smooth and efficient workflow for the entire case management department. What minimum requirements you’ll need Licensure / Certification / Registration: Registered Nurse credentialed from the Texas Board of Nursing obtained prior to hire date or job transfer date required. Registered Nurse obtained prior to hire date or job transfer date required. Education: Bachelor's degree in nursing or diploma from an accredited school/college of nursing required. Master's degree or combination of Bachelor's degree and experience preferred. Work Experience: 5 years of experience required. 10 years of experience preferred. 2 years of leadership or management experience required. 5 years of leadership or management experience preferred.
