Job Title: Case Manager RN Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours Experience: Minimum 1 year experience required License: Active RN license required Certifications: BLS Must-Have: Previous Charge Experience Charting System Experience EPIC Charting Experience Description: This position requires an RN with acute care experience. The RN will cover various acute departments within the facility. Departments may include ICU, medical, med-surg, telemetry, observation unit, and emergency department. The expected patient ratio is 1:16. Weekends may be required as needed. The RN’s role includes coordinating care and assisting patients and families with discharge plans. The RN will work with case management assistants to facilitate authorization and post-acute placements. Support will be provided from an interdisciplinary team. The role does not include utilization review, but knowledge of this area is helpful. Weekend coverage focuses on discharges and new assessments only, without full assignment of cases. Travelers should be prepared to float between departments. A vehicle is required for this assignment. Providers may need to use personal communication devices for work-related tasks.
