Clinical Pharmacist

This job exists to: Provide comprehensive medication management services for patients at clinical locations in collaboration with primary care providers. Specifically, this position provides quality clinical pharmacy services; carries out grant-funded, community-based projects; and partners within Clinical Services to achieve clinical outcomes goals. Examples include coordinating group visit activities, providing Adverse Drug Reactions (ADR) and potential Adverse Drug Reactions (pADR) evaluation, and various pharmacy projects as determined by the clinic administration including further development of clinical pharmacy protocols. Existing Collaborative Drug Therapy Management (CDTM) protocols include but are not limited to: diabetes, hypertension, dyslipidemia, smoking cessation, and anticoagulant management.

In addition, Clinica collaborates with the University of Colorado Skaggs School of Pharmacy & Pharmaceutical Sciences and Regis University School of Pharmacy as an educational training site for pharmacy students and residents. This position provides opportunities for individuals to serve as a preceptor within the scope of clinical services, educational and scholarly activities.

Essential duties and responsibilities include: operating within the clinical scope of supervision and responsibilities as outlined in Clin380.2 Collaborative Drug Therapy Management (CDTM) by a Clinical Pharmacist or most recent version of this policy and procedure; performing comprehensive medication reviews to identify and resolve medication-related problems as requested by primary care providers; providing for the educational needs of healthcare professionals and patients and their families as requested; participating in Pharmacy Committee; ensuring compliance with clinic policies and procedures that apply to clinical pharmacy services; ensuring compliance with all applicable Federal, State and local laws, rules and regulations; maintaining strict confidentiality of patients, guests and employee information; providing quality pharmaceutical services focused on exceptional customer service; managing grant-funded, community based projects, which includes managing funds, identifying and delegating project tasks, and documenting results; building and maintaining high quality business relations with co-workers, clinical affiliations, vendors, clients and other constituents; and managing Adverse Drug Reaction program.

This position is expected to perform responsibilities with general managerial direction. For immediate problems, the position is responsible for independent problem solving and must have the ability to work with minimal supervision. Progress is reviewed quarterly and results are measured and formally evaluated annually.

Position Qualifications: Education and Experience: Graduate of an ACPE accredited School of Pharmacy and must have a PharmD degree. Current Board Specialty Certification in Pharmacotherapy or Ambulatory Care from the Board of Pharmacy Specialties, current certification from the National Institute for Standards in Pharmacist Credentialing, or current certification from the Commission for Certification in Geriatric Pharmacy. Currently licensed to practice pharmacy in state of Colorado. Previous clinical experience in an FQHC preferred. Knowledge, Skills and Abilities: Maintains current pharmacist license. Ensures applicable CE records and licensure are maintained in department files. Attends management meetings. Sustains professional competency and promotes development through participation in workshops, in-services, seminars and professional organizations. Communicates fluently verbally and written in English Fluent in Spanish preferred.
