In this rewarding, 100% remote position, you will serve as a vital link for members managing chronic and long-term care needs. You will perform telephonic outreach, assist members with their care plans, educate them on available benefits, and coordinate with healthcare providers and community partners to address physical, behavioral, and social determinants of health. Working standard Monday through Friday daytime hours (8:00 AM – 5:00 PM EST) with no weekend or overtime requirements, you will enjoy a team culture built on open communication, accountability, and professional growth.

Key Responsibilities:
- Conduct member outreach via phone to discuss care plans, answer questions, and provide ongoing health education.
- Coordinate care activities with healthcare providers, community organizations, and caregivers to accommodate member needs.
- Handle member and provider inquiries regarding service plans and care continuation.
- Support service assessments, screenings, and accurate documentation in compliance with regulatory standards.
- Make appropriate referrals to Care Managers, Disease Managers, and community-based resources.
- High School Diploma or GED required.
- 1–2 years of healthcare experience (physician's office, hospital, or clinical environment preferred).
- High organizational, adaptability, and critical-thinking capabilities.
- Dedicated home workspace with reliable high-speed internet.
- Must reside locally within North Carolina.
