Nurse Practitioner or Physician Assistant - PRN

Role Overview: The provider conducts comprehensive in-home health assessments, reviewing members' health, medical history, medication adherence, and social factors to improve care quality and identify undiagnosed conditions. Key Responsibilities Perform adult/geriatric assessments including medical history, diagnosis, treatment, and health education. Administer and order point-of-care and lab tests as needed. Collaborate with physicians, case managers, social workers, family, and caregivers. Coordinate follow-up and provide patient education in members' homes or facilities. Qualifications & Skills Master’s Degree or equivalent experience with NP licensure; current state licensure with prescriptive authority. Board certification in relevant adult or family practice specialties. Current BLS, ACLS, or CPR certification. Minimum one year NP experience preferred; new graduates encouraged. Valid driver’s license, auto insurance, and computer proficiency. Excellent communication skills.
