You will collaborate with the core PACT Team (Primary Care Provider, RN and Medical Assistant) and expanded PACT Team including family/caregiver, VA, and community-based services involved in developing the patient care plan.Core responsibilities include:Actively diagnosing and treating veterans.Examining patients for symptoms of organic or congenital disorders, determining nature and extent of injury/disorder, and performing comprehensive physical examinations.Developing and implementing patient management plans and assisting in provision of continuity of care.Ordering and performing diagnostic tests.Prescribing medication and recommending dietary and activity programs as indicated by diagnosis.Counseling patients on the use of prescription medications, educating patients, assessing mental health issues, and providing routine health maintenance.Evaluating patients' records from medical providers outside the VA and working with these patients utilizing rules set forth by the VA for co-managed care.Completing any and all clinical reminders "due" at the time of each patient visit.Completing the documentation of the medical record within 24 hours of a patient encounter.Soliciting medical management consult or input when appropriate.Complying with all VA guidelines in regard to appropriate and timely response to all patient request, alerts and notifications, consults, orders, lab results and diagnostic studies.Complying with the VA formulary process and consult protocols.Serving as a collaborative physician to mid-levels where appropriate and needed.Complying with all VA guidelines in regard to appropriate and timely clinical documentation, including the completion of all encounters by close of business.Addressing the clinical needs of all walk-in/triage/urgent care patients.Complying with all VA and company training requirements.Complying with VA requirements related to women's health certification and competencies, as well as any elements needed for maintenance of such certification.Fulfilling compliance requirements of the Office of Inspector General (OIG), Joint Commission (JC), Environment of Care (EOC) oversight, lab compliance and other related items.Remaining focused on achieving excellent clinical outcomes through the specified VA guidelines.Providing excellent customer service to each veteran, both in person and over the phone, as well as to fellow colleagues and clinic visitors.Participating in the clinic's outreach events to help support the clinic's enrollment initiatives.Supporting patient enrollment and retention by providing guidance, when necessary, regarding scheduling, follow-up visits or nurse visits.Embracing and supporting new initiatives, whether clinical or operational, and executing on key strategic initiatives that drive the success of the Valor business.Requirements:Qualifications include:4+ years clinical medical practice; government healthcare environment preferred; specific requirements could vary based on individual VA contract.Doctor of Medicine or Osteopathic Medicine DegreeBoard certified in family practice or internal medicineMust be credentialed and remain in good standing through the Veterans Health Administration (VA).Must comply with and maintain all requirements for a valid, unrestricted license in the state of desired employment, or in any U.S. state or territory, depending on VA contractual requirements.Current certification in Basic Life Support (must be renewed periodically as specified by the certifying agency – AHA valid for two years, e.g.) and in ACLS as specified by individual VAMC contractCurrent, unrestricted Drug Enforcement Administration (DEA) registrationProficiency in written and spoken English.Excellent computer skillsEnergetic and optimistic demeanorStrong service mentality and a focus on achieving all aspects of defined service standardThis is considered a safety sensitive positionFinal compensation will be based on relevant factors including work experience, skills, certifications, and internal equity.Qualified applicants will receive consideration for employment without regard to race, color, religion, sex, national origin, sexual orientation, gender identity, disability, or protected veteran status.
