If you're passionate about making a difference when it matters most, we invite you to join our dedicated hospice care team.Essential FunctionsProvide direct personal care to terminally ill patientsAssist with personal hygiene, grooming, and toiletingSupport safe mobility, including use of walkers and wheelchairsHelp with meal preparation, feeding, and light housekeepingObserve and report changes in the patient's physical or mental conditionOffer emotional and psychological support to patients and familiesMaintain accurate and timely documentation of visitsAttend team meetings and participate in Quality Improvement (QI) activitiesPractice infection control and follow safety protocols at all timesCompensation may vary within the salary range provided based on several factors, including but not limited to a candidate's geographic location, experience, education, skills, licensure, certifications, internal equity, and department equity.
