Summary of the Primary Care Physician role:   The older adults we serve experience challenges with social determinants of health and living, which impact their health. The Primary Care Physician will guide our patients to build their best lives according to their goals. They do this by providing a full spectrum of care while utilizing state-of-the-art technology and establishing trusted longitudinal care in our clinics and via telehealth. Working with other members of the care team, our "person-first" approach strives to create both individual and community wellness.

Our innovative Social Primary Care model is designed to do just that for clinically and socially complex older adultsa population that has long been underserved. This includes "Gather Guides" who connect patients to community resources, and our mobile team of Emergency Medical Technicians and Paramedics who provide hands-on support in the patient's home. They handle post-hospitalization follow-ups, chronic condition monitoring, and home safety assessments, relaying critical data directly back to you.

Minimum Requirements:

• Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics
• An active medical license in the Commonwealth of Massachusetts, or the ability to obtain one prior to start date
• Clinical proficiency in evidence-based primary care
• Ability to communicate and connect with patients from a diverse set of backgrounds
• Proven history of working successfully with EHR and other relevant technologies/tools
• Strong ethics with the ability to navigate through complex and highly sensitive situations with confidentiality
• Desire to be a part of a mission-driven organization with a focus on improving lives and communities
• Ability to work well with others in a non-hierarchical environment where opinions are welcomed regardless of titles
• Ability to remain calm when balancing conflicting priorities

Preferred Experience:

• Experience treating older adults in underserved communities with complex social challenges
