Specialty Case Manager

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient‑centered medical homes, medication‑assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. Specialty Case Manager The Specialty Case Manager provides case management services in the community to individuals who have a mental health and/or substance use diagnosis, survivors of domestic violence, and/or those who are considered high risk due to their acuity level. The role assists in planning, connecting, advocacy, coordination, and monitoring to help patients gain access to health services, financial assistance, housing, employment, education, social services, and natural supports. The Case Manager assesses needs, develops and monitors individualized treatment plans, and documents services provided in an accurate and timely manner. The candidate embraces the recovery philosophy and promotes a safe recovery environment for clients. Responsibilities also include implementing advanced techniques and interventions for high‑risk individuals in the associated program. Skills & Qualifications High school diploma or GED required; associate degree in social sciences preferred. 1‑3 years of experience working with special populations (serious mental illness, survivors of domestic violence, high‑cost/high‑need individuals). 1‑3 years of experience in a behavioral health position providing outreach and engagement activities preferred. Minimum of 1 year of recovery from substance use and/or mental health disorders preferred. Current AZ Driver’s License (valid and in good standing). 39‑month Motor Vehicle Record (MVR).
