Behavioral Health Case Manager

We are excited to open a new location in Dalton GA and are seeking an Behavioral Health Case Manager to join our team. Training and onboarding will take place at 6120 Alabama Hwy Ringgold GA 30736 to provide a smooth transition and ensure you are fully prepared to serve patients at the new site.

General Job Description

The Behavioral Health Case Manager provides support and care coordination services to children, adolescents, and families receiving care within an integrated pediatric primary care and behavioral health setting. The Case Manager works closely with the Behavioral Health Specialist, pediatric medical providers, care team members, patients, parents/guardians, and community service providers to identify needs, address barriers to care, coordinate services, and connect patients and families with appropriate resources.

The Case Manager assists with identifying patient strengths, needs, goals, and barriers and works collaboratively with the patient, family, and clinical team to develop and implement individualized care plans.

The Behavioral Health Case Manager may also be responsible for ancillary duties and tasks that support the overall operations of the Behavioral Health and Pediatric programs. These duties will be assigned based on the needs of the department and organization and may change as program needs change.

The Case Manager does not function as an independently licensed behavioral health clinician and does not provide services outside the scope of their education, training, experience, and assigned responsibilities.

Qualifications

• Ability to read, understand, and apply information related to behavioral health, healthcare, social services, community resources, and organizational policies.
• Demonstrates strong verbal and written communication skills.
• Ability to communicate effectively with children, adolescents, parents/guardians, healthcare providers, community agencies, and other service providers.
• Ability to establish and maintain professional relationships with patients and families.
• Ability to work independently while also functioning effectively as a member of an interdisciplinary team.
• Ability to organize, prioritize, and manage multiple responsibilities.
• Ability to maintain confidentiality and follow organizational policies and procedures.
• Demonstrates sound judgment and the ability to recognize when a concern should be escalated to the Behavioral Health Specialist, medical provider, supervisor, or other appropriate professional.

Education & Work Experience

Education:

• Bachelor's degree from an accredited college or university in Social Work, Psychology, Human Services, Sociology, Family Studies, or another closely related behavioral health or social service field.

Work Experience Required:

• Required experience working in behavioral health, social services, healthcare, case management, community services, or a closely related field.
• Experience working directly with children, adolescents, families, and/or individuals with behavioral health or psychosocial needs strongly preferred.
• Experience navigating community resources and social service systems preferred.
• Experience coordinating services with schools, behavioral health providers, healthcare providers, or community agencies preferred.
• Experience working with families experiencing social, economic, behavioral health, or other barriers to care preferred.

Skills/Knowledge Required

• Knowledge of behavioral health, social service systems, community resources and referral processes.
• Understanding of the social determinants of health and ACE's and their impact on children and families.
• Understanding of basic behavioral health concepts and the ability to recognize behavioral health concerns that require referral or clinical intervention.
• Strong organizational and time-management skills.
• Strong communication and follow-up skills.
• Ability to document patient contacts, referrals, progress, and services accurately and timely.
• Ability to work with electronic health records and basic computer systems.
• Ability to maintain accurate records and track referrals and patient needs.
• Ability to advocate for patients and families while maintaining appropriate professional boundaries.
• Ability to work effectively with multidisciplinary healthcare teams and community partners.
• Ability to maintain appropriate confidentiality when working with patients, families, and outside agencies.
• Bilingual English/Spanish Strongly Preferred
