Physical Therapist PRN

Job Summary:

Glenbridge Health and Rehabilitation of Boone, NC, is seeking a highly skilled and dedicated Physical Therapist (PT) to join our team on a PRN basis. Physical Therapist Key Responsibilities:

• Conduct comprehensive physical therapy evaluations and develop personalized care plans based on clinical findings and resident goals

• Deliver physical therapy treatments as prescribed, including therapeutic exercises, gait training, balance activities, and functional mobility

• Monitor and document residents’ progress, response to treatment, and adjust plans as necessary

• Collaborate with an interdisciplinary team including nursing, occupational therapy, speech therapy, and physicians to provide holistic care

• Educate residents, families, and caregivers on therapy programs, techniques, and safety measures to support functional independence

• Comply with all regulatory, licensing, and facility policies including documentation and infection control standards

Physical Therapist Qualifications:

• Graduate of an accredited Physical Therapy program

• Current and valid Physical Therapist license

• CPR certification required (or must obtain upon hire)

• Prior experience in a skilled nursing or long-term care setting preferred

• Strong communication, clinical, and organizational skills

Additional Requirements:

• Minimum of 1 year of experience as a Physical Therapist

• Ability to travel to and from work independently as needed

• Availability to work variable shifts, including days, evenings, and weekends
