As a Home Health Registered Nurse, you'll provide skilled, individualized care while developing meaningful relationships with patients and their families. You'll have the opportunity to educate, encourage, and advocate for your patients while seeing the direct impact of your care every day.

What You'll Do

• Perform comprehensive nursing assessments and skilled visits
• Develop, implement, and update individualized plans of care
• Provide skilled nursing services, including medication management, wound care, disease management, and patient education
• Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments as appropriate
• Monitor patient progress and communicate changes to physicians and the interdisciplinary care team
• Coordinate care with therapists, social workers, and home health aides
• Complete timely and accurate clinical documentation using WellSky
• Educate patients and caregivers to promote safety, independence, and improved health outcomes

Qualifications

• Active Washington, DC Registered Nurse (RN) License Required
• Valid Driver's License and Reliable Transportation
• Strong Clinical Assessment, Critical Thinking, and Documentation Skills
• Home Health Experience Preferred but Not Required
• OASIS Experience Preferred
• Experience with WellSky Home Health is a plus
• Passion for Delivering Exceptional Patient-Centered Care
