Long-Term Care/Special Needs Registered Nurse (RN)

We are seeking a dedicated Long-Term Care/Special Needs Registered Nurse (RN) to join our team. This position involves providing compassionate care to residents in a Long-Term Care (LTC) and Skilled Nursing Facility (SNF) setting. The ideal candidate will have experience in geriatrics or psychiatric care, particularly with the elderly population, including those with dementia and behavioral health needs.

Key Responsibilities

• Administer medications and treatments per established protocols.
• Conduct comprehensive patient assessments and develop care plans.
• Monitor patients' conditions and document any changes in the Electronic Medical Record (EMR).
• Collaborate with interdisciplinary teams to ensure holistic patient care.
• Provide education to patients and families regarding health management and disease prevention.
• Assist in behavioral programs for patients with psychiatric diagnoses.
• Participate in quality improvement initiatives and staff meetings.

Qualifications

• Current Registered Nurse (RN) license.
• Basic Life Support (BLS) certification is required.
• Minimum of one year of nursing experience, preferably in a geriatric or psychiatric setting.
• Strong communication and interpersonal skills.
• Ability to work effectively with a diverse population of residents.
• Willingness to work 12-hour shifts, including every other weekend and holidays.

Work Environment

This position is located in a Long-Term Care/SNF facility dedicated to providing quality care to a senior population. Nurses will have support from medication aides and collaborate with a dedicated team to ensure comprehensive care.

Additional Information

• Common diagnoses include dementia as well as behavioral health conditions such as schizophrenia and bipolar disorder.
• Orientation and training will be provided, including opportunities to shadow experienced staff members.
• This is a local position, with anticipated working hours from Sunday through Saturday.
