Public Health Nurse/Registered Nurse

As a Public Health Nurse/Registered Nurse within the Community Health Care Services (CHCS) Department, this role is instrumental in administering community health care programs tailored to meet the unique needs of United Indian Health Services' (UIHS) client population. Working under the guidance of the CHCS Manager, this position will oversee clinical duties performed by Community Health Representatives (CHRs), provide direct care to clients, and contribute to broader objectives aimed at promoting community health and wellness. Embracing the cultural and traditional values inherent to American Indians and Alaska Natives, this position will ensure the delivery of exceptional health care services and the advancement of wellness initiatives within the community. The planning and implementation of all activities within this role are intricately aligned with these foundational principles, ensuring that programs and interventions are culturally sensitive and respectful.

Essential Duties and Responsibilities:

Level I

• Conduct comprehensive health assessments of individuals and communities to identify health needs and risk factors.
• Provide direct nursing care, including screening, vaccinations, and treatments, in various settings such as clinics, homes, and schools.
• Develop and implement health promotion and disease prevention programs tailored to community needs using culturally sensitive and evidence-based practices.
• Maintain accurate documentation of client interactions, interventions, and outcomes in accordance with regulatory standards.
• Provide comprehensive training for CHRs on essential tasks, including home visits, vital sign assessment, referral procedures, and documentation.
• Deliver skilled public health nursing services through referrals, assessments, individualized care plans, and treatments under healthcare provider directives.
• Review and analyze documentation and charting completed by CHRs to ensure the provision of quality care and adherence to established protocols and standards.
• Adhere to Health Insurance Portability and Accountability Act (HIPAA) and relevant regulations for client confidentiality.
• Coordinate case management activities, prioritizing client advocacy and support while maintaining meticulous records of care interventions and fostering effective communication with UIHS staff.
• Work closely with CHRs to collaboratively develop, implement, and evaluate individualized care plans for clients, ensuring comprehensive and effective support.
• Support the coordination of high-quality immunization programs for clients of the UIHS, ensuring efficient and effective delivery of vaccination services.
• Participate in departmental and designated meetings, actively contributing insights and ideas to enhance program development and coordination efforts.
• Uphold professional standards, organizational policies, and standard operating procedures (SOPs) to ensure a conducive and safe work environment.
• Must be willing to learn more about the American Indian culture and attend at least one Cultural Presentation/Training per year.
• Must be willing to work with American Indian/Alaska Native people and demonstrate cultural awareness and humility.
• Must be willing to deliver services in a culturally responsive manner.

Level II

• This position will complete all the assigned tasks outlined in the Clinical Nurse I category, in addition to the tasks outlined for the Clinical Nurse II category.
• Assist in the preparation and submission of grant/funding applications while ensuring compliance with reporting requirements to facilitate the financial sustainability and accountability of the organization.
• Lead investigations and interventions in response to outbreaks of communicable diseases within the community, collaborating with relevant stakeholders to mitigate spread and ensure timely interventions.
• Contribute to the development of comprehensive quality improvement activities, actively participating in program reviews and compliance assessments to enhance service delivery standards.
• Assist in the preparation of special studies and research programs, leveraging clinical expertise to inform evidence-based practices and policy development.
• Support the development of departmental policies, procedures, and quality improvement initiatives within the clinic, fostering a culture of continuous learning and enhancement.

Supervisory Responsibilities:

• This position will assist the Community Health Care Services Manager with overseeing the clinical duties performed by the Community Health Representatives (CHRs).

Education/Experience: Educational degrees must be from a US Department of Education accredited school

Level I

• RN-Possession of a valid Registered Nurse California license.
• Bachelor of Science in Nursing, Microbiology or Epidemiology or Public Health and/or public health nurse specialty certificate issued by the state of California.

Level II - In addition to Level I requirements.
