In this role, you will complete skilled nursing visits, perform start-of-care and routine assessments, and support patients throughout the Western North Carolina home health area. Job Requirements

• Active North Carolina RN license or compact RN license
• Associate degree in Nursing required
• OASIS experience required
• HomeCare HomeBase (HCHB) experience is a plus
• Strong communication, organization, and time-management skills
• Ability to work independently while maintaining excellent patient care standards

Responsibilities

• Provide skilled nursing interventions in patients' homes
• Complete start-of-care, routine, evaluation, and other home health visit types as needed
• Assess patient condition, document care accurately, and update care plans accordingly
• Coordinate with the interdisciplinary care team to support safe, effective care delivery
• Manage scheduled on-call coverage as assigned
• Travel between patient visits within the designated coverage area
• Deliver compassionate, patient-focused care that supports recovery, independence, and continuity of care at home
