Home Health Speech-Language Pathologist (SLP)

Location: Mount Angel, OR and Surrounding Areas

Assignment Duration: This is a 13-week contract position.

Schedule: 3x8-hour Days, Wednesday to Friday, 08:30 AM - 05:00 PM

Job Description: We are seeking a dedicated Home Health Speech-Language Pathologist (SLP) to provide care for patients in their homes. This role involves conducting assessments, creating individualized treatment plans, and delivering therapeutic interventions to patients with speech and language disorders.

Responsibilities:

• Conduct evaluations and develop treatment plans based on individual patient needs.
• Implement therapy interventions to address speech and language disorders.
• Document patient progress and adjust care plans as needed using OASIS documentation.
• Collaborate with interdisciplinary teams to ensure holistic patient care.

Required Experience / Certifications / Licensure:

• Minimum of 1 year of skilled home health experience required.
• BCLS (Basic Cardiac Life Support) certification is required.
• Experience with OASIS documentation is mandatory.
• Familiarity with the EPIC charting system is preferred.
