Salary: $110,000 - $130,000 annually Position Overview Wheeler Staffing Partners is seeking an experienced Territory Clinical Manager (RN) to lead and oversee clinical operations within a designated home health territory. This role is responsible for ensuring high-quality patient care, supervising clinical staff, and maintaining compliance with regulatory standards.
The ideal candidate brings strong leadership experience in home health, a passion for patient-centered care, and the ability to manage clinical teams in a fast-paced, field-based environment. Key Responsibilities

• Provide clinical leadership and oversight for home health services within the assigned territory
• Supervise, mentor, and support clinical staff including RNs and field clinicians
• Ensure delivery of high-quality patient care in compliance with regulatory and organizational standards
• Monitor clinical performance, patient outcomes, and quality improvement initiatives
• Coordinate care plans and collaborate with interdisciplinary teams
• Manage scheduling, staffing, and productivity within the territory
• Conduct field visits and maintain visibility across service areas
• Ensure proper documentation, chart audits, and adherence to compliance requirements
• Support onboarding, training, and development of clinical team members

Qualifications

• Active Registered Nurse (RN) license in the state of California (required)
• Minimum 1 year of home health experience (required)
• Minimum 1 year of supervisory, management, or lead experience (required)
• 3-5 years of RN experience in home health, community, or clinical settings
• Experience managing clinical services and staff in a home-based care environment
• Strong leadership, communication, and organizational skills
• Ability to travel throughout the assigned territory

Certifications & Requirements

• Current RN licensure in California
• Valid CPR certification (must be maintained)
• Valid driver's license and reliable, insured transportation
• Ability to commute within the assigned territory
