Schedule
- Shift: 8-hour days

Assignment Details
- Visits: 1:1; standard target 30 productivity points; may visit up to 6 patients/day (typical = 5)

Requirements
- License: Oregon RN licensure
- Experience: Minimum 1 year skilled home health RN experience
- Certifications: BCLS
- OASIS experience: Required

Roles & Responsibilities
- Conduct start of care and admission assessments for home health patients.
- Provide routine visits, recertification, and discharge planning.
- Perform wound care, including Wound VAC management.
- Manage G-tube and colostomy/ileostomy care.
- Administer IV/infusions, IM/ID/SubQ injections, and manage medication administration.
- Assess and manage patient pain and provide education on catheter care.

Compliance / Notes
- Orientation: 40 hours
- Parking: Free
- Dress Code: Business casual preferred; scrubs allowed (no jeans; no specific scrub color)
- Mileage reimbursement: IRS standard
- Potential for facility to call off up to one shift per pay period.
