Remote RN - Personal Care Services Assessor (Medicaid)

Vaya Health

Vaya Partners seeks a Registered Nurse to perform Personal Care Services assessments for Medicaid beneficiaries in North Carolina. The role is remote but requires residency in NC and travel within specified counties to complete in-home and community-based assessments.

The assessor will schedule appointments, conduct telephonic and in-person evaluations, submit SARs, and coordinate with healthcare professionals while upholding privacy and dignity. Requires NC RN licensure and NC residency.
