Nurse Practitioner - Exempt

Summary/ObjectiveProvides medical services within the scope of licensing, training, and credentialing, in accordance with specialty for patients of all ages, under the supervision of the Physician. Must meet credentialing requirements for NSHC, as applicable, and must demonstrate clinical competency in the area of Medicine.Essential FunctionsAssessment of health status:Obtains a relevant health and medical historyIdentifies medical health risks and needsUpdates and records changes in health statusFormulates Diagnosis and implements appropriate treatment plan designed to maximize the health of patients:Orders appropriate diagnostic testsDetermines treatment plan of carePrescribes appropriate pharmacological agentsPrescribes non-pharmacological agentsDevelops a client education planMakes appropriate referrals to other health professionals and community agenciesMaintains a process for systematic evaluation of client status by:Scheduling return appointments for follow-up care, as necessary, according to best practice standards of careDetermining the effectiveness of the plan of care through documentation of client outcomesReassessment and modification of care plan as necessary to achieve goalsSeeks to promote maximal health among clinic patients by:Providing educational opportunities for each patientIncorporating patient’s input in the development of the treatment planProviding information needed to make health care decisions related to appropriate utilization of health care resources.Responds to tasks and completes charting in a timely manner to prevent any delay in care and promote coordination of care.Participates in quality assurance reviews, peer reviews, etc. on a periodic basis, including systematic review of records and treatment plansPrepares reports and other documentation as requiredAttends meetings and training sessions as requiredParticipates as a team member in the provision of medical and health care bycollaborating with other care professionals to provide comprehensive careExecutes other verbal or written specific assigned tasks, requiring similar or lesserskills and abilities, some of which are continuing while others are occasional innature.These essential functions are a summary of the primary duties and responsibilities of the position, and are not intended to be a comprehensive listing of all duties and responsibilities. Work may include hand dexterity as well as the need to reach, climb, balance, stoop, kneel, crouch, talk, and hear. The employee must occasionally lift and/or move up to 50 lbs. While performing the responsibilities of the job, the employee is required to talk and hear. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to focus.
