Minimum Requirements

• Master's (MPT) or Doctor of Physical Therapy (DPT) degree from an accredited program.
• Active Colorado Physical Therapy license required to start the assignment.
• Home health experience with OASIS documentation required.
• Experience performing comprehensive patient evaluations, developing measurable plans of care, and documenting skilled necessity for reimbursement.
• Proven ability to provide skilled PT interventions in home settings, including therapeutic exercise, gait and balance training, transfer and stair training, and assistive device training.
• Experience educating patients and caregivers on home exercise programs, fall prevention, energy conservation, and caregiver transfer techniques.
• Experience coordinating with interdisciplinary teams and communicating with physicians regarding plan of care and discharge planning.
• Daily travel to patient homes within an approximately 50-mile service radius. Ability to travel to multiple locations within a shift required.
• Preferred: Experience with HCHB electronic documentation system.
• Certifications: BLS from AHA or ARC may be required.

Assignment Details

• Facility Type: Home Health.
• Shift Name: Days.
• Shift Details: Days 0800-1600.
• Work Days Per Week: Monday through Friday, five days per week. No call.
• Travel Requirement: Daily travel to patient homes within the agency service area; may visit multiple patient locations per shift.
• Dress Code: Scrubs in blue or black.
• Interview Process: Phone or Microsoft Teams.
• Education Requirement: Minimum Master’s degree.
