Director of Community Relations Full-time Brunswick, GA, US 2 days ago Requisition ID: 1005 FirstLight Home Care of Southeast Georgia is seeking a dynamic, energetic, and relationship-driven Community Outreach & Intake Coordinator to help grow our presence in the community and connect families with the quality home care services they need. This position is responsible for building and maintaining relationships with referral sources, generating qualified leads, coordinating client intakes, and supporting the growth of the agency. The ideal candidate is highly organized, professional, comfortable meeting new people, and passionate about building long-term relationships. Position Responsibilities Community Outreach & Referral Development Build and maintain strong relationships with referral sources through face-to-face visits, phone calls, presentations, meetings, networking events, and other community outreach activities. Seek out and develop new referral relationships to increase client referrals and agency growth. Regularly visit existing referral sources to maintain and strengthen partnerships. Attend community events, speaking engagements, volunteer opportunities, and other networking activities to increase brand awareness and generate leads. Develop and maintain an active networking plan and regularly review and revise the plan as needed. Track networking and outreach efforts and report activities and results to the immediate supervisor on a weekly basis. Work collaboratively with the FirstLight Home Care team to monitor goals related to billable hours, client growth, referrals, and revenue. Maintain and update prospect, client, and referral source databases, including contact and mailing lists. Develop consistent communication and follow-up with prospects and referral partners through multiple outreach channels. Intake Coordination Serve as a point of contact for prospective clients and families inquiring about FirstLight Home Care services. Coordinate the client intake process from initial inquiry through the start of services. Gather required client information and assist with scheduling consultations, assessments, and other intake-related appointments. Communicate with prospective clients, families, referral sources, and internal office staff throughout the intake process. Work closely with the Office Manager, Scheduler, and other team members to ensure a smooth transition from referral to active client services. Maintain accurate and timely intake documentation and follow-up records. Provide professional, compassionate, and responsive customer service to prospective clients and families. Qualifications College degree in Business, Marketing, Health Care, or a related field strongly preferred, along with at least two years of related experience and/or training. Experience in home care, health care, senior services, or another service-based industry preferred. Excellent communication, interpersonal, presentation, and relationship-building skills. Ability to represent FirstLight Home Care with a positive, professional, and polished business image. Strong organizational, time-management, and follow-up skills. Comfortable working independently while also collaborating with an office team. Proficient with computer systems, databases, email, and standard office software. Valid driver's license and reliable transportation required for community outreach and referral visits.
