Responsibilities Completes an initial home health assessment of patient and family to determine home care needs; provides a full physical assessment and history of current and past illnesses. Utilizes the nursing process in planning, providing, and evaluating skilled needs. Completes a comprehensive patient assessment reflected in accurate OASIS scoring. Develops a care plan with goals based on nursing diagnoses and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes patient and family in planning and revising care. Requirements Graduate of an accredited school of nursing. At least 1 year of recent home Care experience Ability to exercise initiative and independent judgment. #J-18808-Ljbffr T E C Industrial Inc
