CART Screener (Child Adolescent Response Team)/

ECSS Screener/Resource Navigator (Early Childhood Support and Services)

Summary

The Screener/Navigator is responsible for providing assistance to families engaging in services from initial screening and assessment of needs through ongoing connection to community resources and supports. The duties of this position are entry level, with direct supervision from an experienced Licensed Mental Health Professional.

Essential Functions

· Conduct initial screenings with individuals seeking crisis services through phone, in-person, or virtual assessments.

· Conduct initial ECSS screenings with parents who call, present, or are referred to the clinic.

· Serve as a back-up to routine, clinical screeners

· Collect and document relevant client information, including demographics, presenting concerns, mental health history, environmental risks factors, and other social determinants of health.

· Collect, organize, and compile necessary forms.

· Use standardized screening tools to assess symptoms, risk factors, needs and service eligibility.

· Ensure initial screening documentation is completed accurately and entered into the electronic health record.

· Work independently and collaboratively to locate and stay abreast to community resources and supports, keeping clinical team informed and working with clinical team and directly with families to connect families to resources and supports to meet their needs.

· Educate individuals and families about resources and support systems available to them.

· Promote hope, personal responsibility, empowerment, education, and self-determination in all aspects of their work

· Coordinate referrals for internal and external services, ensuring individuals are matched with the appropriate level of care.

· Collaborate with clinicians, case managers, and administrative staff to facilitate the intake and treatment process.

· Handle routine administrative duties, including filing, scanning, copying, and faxing.

· Maintain accurate and confidential records in compliance with HIPAA, 42CFR and clinic policies.

· Assist Clinic/Unit Manager(s) with ongoing specialized assignments, complex projects and other duties as directed

· Be available to assist and dispatch out for crisis calls

Required Education and Experience

Bachelor’s Degree in a social Service Field and 1-3 years of experience is required. Working knowledge of computers and software is required. Must possess a Louisiana driver’s license and have access to an insured automobile.

While performing the duties of this job, the employee is frequently required to stand; walk; sit, use hands, handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; taste or smell. The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. While performing the duties of this job, the employee can sometimes be exposed to weather conditions. The noise level in the work environment is usually moderate.
