Case Manager (Job ID: 110609) Location: Monaca, Pennsylvania, United States Position Type: Full-Time Shift: 1st Shift – with week-ends Work Schedule: Monday‑Friday 8 am‑4:30 pm; one 12 pm‑8:30 pm shift a week; rotating on‑call, weekends and holidays Requirements Bachelor's Degree in a human service field 1 year experience working with individuals in a recovery setting and/or crisis experience Valid PA Driver's License. Travel necessary across Beaver County. Position Details Provide rehabilitation, support, and direct care services to individuals with mental health and behavioral health needs, including assistance with daily living activities and therapeutic interventions. Implement treatment plans under clinical supervision, including substance use services, education, family consultation, and crisis intervention support as needed. Collaborate with families, healthcare providers, social service agencies, and community partners to coordinate comprehensive care and support positive outcomes. Ensure the safety and well‑being of individuals served by following established policies, procedures, and safety protocols. Participate in treatment planning and work collaboratively with the interdisciplinary team to support recovery and treatment goals. Special Conditions Actively participate in Assertive Community Treatment (ACT) Teams, which may involve at least three home visits per week.
