Field RN - Home Care Location: Rockland & Orange Counties, NY. Schedule: Full-Time. Compensation: $55-$60/hour. Benefits: Medical, dental, vision, and additional benefits. A growing home care organization is seeking a Field RN to conduct in-home assessments and develop individualized plans of care for patients throughout Rockland and Orange Counties. This is not a skilled nursing position. The role focuses primarily on assessments, care planning, documentation, and coordinating appropriate services based on each patient's needs. Responsibilities Conduct in-home patient assessments. Develop and update individualized plans of care. Evaluate patient needs and recommend appropriate services. Complete required documentation and follow-up. Communicate with patients, families, and care teams. Ensure care plans remain appropriate as patient needs change. Qualifications Active New York State RN license required. Home care experience preferred. Strong assessment, documentation, and communication skills.
