Are you a compassionate Registered Nurse or Licensed Practical Nurse passionate about helping individuals with serious mental health challenges achieve recovery and independence? We are seeking a dedicated PACT Nurse (RN or LPN) to join our multidisciplinary Assertive Community Treatment (PACT) team in Quincy, MA.
What You'll Do
As a PACT Nurse, you'll play a vital role in delivering comprehensive, community-based behavioral health services by:
- Conducting psychiatric and physical health assessments.
- Developing, implementing, and monitoring individualized treatment plans.
- Administering and monitoring medications, including evaluating effectiveness and side effects.
- Providing case management, supportive counseling, and symptom management.
- Collaborating with psychiatrists, medical providers, families, and community partners.
- Supporting individuals in achieving housing, employment, wellness, and recovery goals.
- Responding to behavioral health crises and providing intervention as needed.
- Participating in team-based treatment planning and care coordination.
- Maintaining accurate clinical documentation and progress notes.
Qualifications
Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Massachusetts.
Minimum of 2 years of experience in behavioral health, psychiatric nursing, or community mental health.
Strong assessment, communication, and crisis intervention skills.
Experience working with individuals experiencing serious mental illness and co-occurring substance use disorders preferred.
Ability to work independently while contributing effectively to a multidisciplinary team.
